Limited Warranty Information

Subject to the return of the restoration or appliance that was accepted, placed, and then ultimately failed, PFD Digital Lab will repair or replace it without charge for the cost of materials and workmanship as follows:

  • PFD Posterior Zirconia (lifetime period)

  • PFD Anterior Zirconia (lifetime period)

  • PFD Layered Zirconia (five year period)

  • Porcelain Fused to Metal (five year period)

  • Full Cast Crown (five year period)

(Exceptions detailed in our "Remake and Refund Policy")

  • Screw Retained Implants (five year period)

  • Custom Implant Abutments (five year period)

  • IPS e.max, Empress (two year period)

  • Night Guards, Retainers (one year period)

  • PMMA Temporaries (two month period)

Remake and Refund Policy

PFD Digital Lab warrants that all custom dental prosthesis and appliances are made according to your requirements as specified by your paper or digital prescription, and fits the cast provided by your impression or digital impression. As a general rule, if the prosthesis or appliance we manufacture fits the model, but not the mouth, then there is a discrepancy in the impression or digital impression provided to us, and thus PFD Digital Lab will not be responsible for the cost incurred to remake or replace the prosthesis or appliance. We do make exceptions to this general rule as a gesture of appreciation and goodwill. Our main goal is to determine the root causes of failed work and strive to reduce or eliminate those causes. We will often suggest to our partners different procedures and workflows, bring to light possible oversights, and generally work towards creating a trusting environment where the outcome moves towards reducing remake instances. We are here to be your partner in delivering successful outcomes for your patients. Our overall remake rate for year end 2019 was 1.2%, well below the national average of 3.8%[1]

 

The cost for fabricating custom made dental appliances cannot be refunded. A credit may be issued by PFD Digital Lab directly to a customer’s account when cases do not meet our standards for quality, function and aesthetics. Any credit balance on an account must be used for lab services within 60 days from the date of issue or it will expire. We reserve the right to waive our remake exception requirements on a case by case basis. Remakes will be no cost if received within six weeks of the invoice date, with the following exceptions listed below:

 

  • We inquired about the quality of the impression or digital impression and requested a new impression or digital impression be sent. We were told by the doctor or doctor's staff to continue with the original. 

  • The doctor trimmed or digitally marked the die resulting in an open margin.

  • Bridges where the pontic unit(s) had not been extracted or allowed sufficient healing time prior to the impression being taken.

  • The doctor instructed us to adjust the opposing due to a lack of clearance, or adjust the abutment due to a lack of clearance, lack of parallelism, undercut, or other reason.

  • Any crown fabricated under a partial/designed to an existing partial, where the actual partial is not provided to us.

  • The remake case instructs for a shade that is different than the original.

  • The remake case instructs a design change different than the original.

  • The remake cases' abutments have been re-prepared, adjusted, or altered in any way from the original case.

  • Remakes returned to us without the original restoration, impression, and model will be charged at 50% of the price. If the original restoration is returned to us at a later date, we will issue a credit to the doctor’s account.

Payment Terms

PFD Digital Lab offers you the convenience of a monthly open account (net 30) on all of your casework. Invoices are sent with cases and statements are mailed on the last business day of the month. The statement balance is due and payable by the end of the following month. A service charge of 1.5% per month (annual rate of 18%) will be applied to any unpaid balance. Any costs associated with collecting an unpaid balance, including legal fees, will be charged to the customer. We reserve the right to limit or terminate credit due to unpaid and/or untimely payment of statement balances, and convert accounts to COD status. We accept payment by check, and bank or credit card (Visa, MasterCard, Discover, and American Express). You can setup to be billed automatically with a card on file by contacting us.
